HIV Testing in the Philippines – How to get tested.

The Department of Health now ranks Philippines as one of the countries that has the fastest growing HIV epidemic in the world with a prevalence rate of 5% nationwide. There is a 20-25% prevalence rate in Metro Manila and 15-20% prevalence rate in Cebu. This rate dwarfed the prevalence rate in South Africa, which has the largest population of HIV positive individuals in the world, with a mere 4.7%. Prevalence rate refers to the percentage of individual affected by the disease at specific time. With that in mind, we can say that right now, in a population of 110 Million Filipinos, there are 5.5 Million individuals currently living with HIV.

Mere 5 years ago when I started making this blog, The Philippines is a low prevalence country with less than 1% of the population affected by HIV. This rapidly growing epidemic if left uncontrolled will eventually make The Philippines at par with South Africa with 1 out 10 individuals affected with HIV. So what went wrong? As a nurse with 15 years experience handling HIV cases and working actively with HIV positive individuals, here are my personal conclusions, do take note that this conclusions are not even close with the official conclusion of the Department of Health on why HIV became a rapidly growing epidemic in The Philippines. I personally think that their conclusion is too traditional and not in touch with the modern activities of our youth.

  1. DOH says that the resistance of the Church to some forms of birth control is a contributing factor on the HIV epidemic right now. I say that HIV is most commonly seen in the young population. A population that makes their own decision and is rebellious to the church teachings. Though the Church could have cause some sort of contribution, I do believe that the lack of sex education in our school particularly to our youth is a more blameable culprit than the Church. Did you know that people ages 14 to 30 makes up the 95% of newly diagnosed HIV cases each month and the median age of affected individual for the last 5 years is just 28 years old?
  2. DOH says prostitution or sex for money is a contributing factor of the epidemic and should be eradicated. I say that, prostitution cannot be eradicated. Since the dawn of human civilization, there is prostitution and it is something that is very hard to control much less eradicated from our society. What DOH can do is to provide an ID system and free monthly checkup to our sex workers to protect them as well as the people that they serve. This will also encourage the client to only have sex with a “clean” sex worker. This will also further educate our sex workers on proper ways of protecting themselves from different sexually transmitted infection as well as unwanted pregnancy.
  3. DOH says that persons under 18 who wants to test for HIV should require parental consent. I say that, I have clients with HIV at age 13, 14, 15… and that is because of failure in health promotion. The accessibility of casual sex thru apps like Grindr and other dating apps makes it very easy for 2 complete strangers to have sex consensually and thus, spreading the virus like wildfire to our young and techie population. These individuals would never dare to ask their parents permission to go with them and get tested, our culture against premarital sex and our bigotry against the LGBT makes it impossible to do so. HIV testing should be accessible to ANYONE at ANY AGE on their own volition.

THE CURRENT STATISTICS (2017)

It is estimated that there 1,000 reported HIV cases per month since the 6 months of 2016. At that rate, we can assume that there are 30 to 50 reported HIV cases daily. This does not include unreported cases. Unreported cases are HIV positive individuals who do not know they are infected. The epidemic growth rate is about 200% per year. This means that if not controlled, we could see a tenfold growth of HIV cases (300-500 cases per day) within the next 10 years.

HOW TO GET TESTED: HIV TESTING IN THE PHILIPPINES

One of the most important thing to do to prevent transmission is knowing your status. HIV is a very controllable disease as long as it is detected early and has not yet done major damage to your organs most specially the Liver, Kidneys and the Brain. There are many ways of getting tested for HIV in the Philippines. We will discuss each of them, one by one including their advantages and disadvantages.

I. Hospitals

Testing by going to hospitals is one of the most common means of getting tested. There are 5 hospitals in our country that specializes with HIV counseling and testing as well as referrals in case you tested positive and needing further treatment. In no particular order: RITM, San Lazaro Hospital, St. Lukes Medical Center both in Quezon City and Global, Makati Medical Center and Medical City in Ortigas. Do take note that the cost for HIV testing is around 1,100 to 3,800 in the private hospitals I mentioned.

PROS: If done correctly, results are very accurate. This is also the only results acknowledged in various employment and immigration requirements.

CONS: There is always an issue with regards to privacy and confidentiality getting tested  in any hospital. Though privacy is protected by our health care professionals, there are many incidents of information leakage caused by poor protocol with regards to transfer of information such as students getting free access to medical records without permission or a slip of tongue during a conversation of one of the member of the health care team handling the HIV case. Hacking is another risk when it comes to electronically stored medical information and records of medication being dispensed from the Pharmacy and to whom.

When it comes to public hospitals, though uncommon, there are recorded cases of results being switched between patients. It is not unheard of that a certain person tested positive though in reality, he is negative simply because a specimen sample was incorrectly labeled with his name. Lapse in judgment and concentration of our overworked and underpaid medical professionals is the main culprit in most cases.

PROS: Free and they offer counselling as well. Staff are very friendly, professional and trained.

CONS: Privacy and result confidentiality are concerns when it comes to free and public testing. Though trained, innocent conversations could lead to information leakage both consciously and unconsciously. There are cases wherein a Volunteer saw a recently positive patient in the mall and told his friend that “That guys just tested positive yesterday.”

III. HIV testing kits

Advanced home test kits like Fujibio HIV kit has 99.9% accuracy similar to that of Hospital based testing.

Testing kits are widely used as the main method in HIV detection in some hospitals and hygiene clinic. All free testing done by volunteer groups uses HIV test kits. HIV testing hubs also uses HIV kits. Medical missions and workplace testing will usually use HIV testing kits. They are easy to use with result at 99.9% accuracy seen in mere 10 minutes. The common brands in The Philippines are Fujibio and SD Bio. Fujibio is Japanese made and Japan FDA approved while SD Bio is Korean made and Korean FDA approved. Both should give an accurate result with 99.9% certainty similar to that of hospital based testing. PROS: Very easy to use and results are very accurate. FDA Japan Approved. Do note that here in The Philippines, FDA does not approve any home test kits to encourage hospital only testing. I would personally recommend testing using test kits first as to give you an accurate idea of your status before going to the hospital. That way, you will know that you will test negative or positive in advance privately. It is also very convenient and you are assure that only you will know the result. CONS: Since the person that bought the kit will usually test himself or herself, Anxiety could lead to testing delay. Some people will contemplate for weeks before using the test kits and then eventually forgets that he bought one.

I TESTED POSITIVE. WHAT SHOULD I DO NEXT?

If you used any of my recommendations above in getting tested and you tested positive, DO NOT DESPAIR. HIV is a very controllable disease and achievement of undetectable viral load is the treatment goal. Living a normal life is very possible with HIV as long as treatment is started and adherence to the treatment regimen is established. We have 2 options in getting treated.

Research Institute for Tropical Medicine (RITM)

RITM is the premiere and the leading facility in The Philippines when it comes to antiretroviral drug distribution as well as treatment for HIV patients. This should be your first choice in obtaining confirmatory results and further treatment if the initial testing result was positive. RITM is more specialized than San Lazaro and could deal with serious HIV related complications such as Mycobacterium Bovis infection as well as cryptococcal infection of the brain.

For commuters:

  1. Take a bus or van going to Alabang.
  2. Look for the Muntinlupa Fire Station.
  3. Ride a UV express/van or a multicab with a Filinvest Ikot route in front of the Orange Building and Festival Mall just besides the fire station.
  4. Tell the driver to drop you off at RITM.

Total estimated fare cost: 60 -120 Pesos.
For private vehicles:

  1. Proceed to SLEX.
  2. Exit at Filinvest.
  3. Turn left after the toll gate.
  4. RITM is just at the right side.

Process:

  1. Ensure that you arrive before 7:00 A.M for baseline testing. You will not be entertained if you arrive late.
  2. Once inside, simply proceed to the ARG clinic.
  3. Register.
  4. Proceed to the cashier and pay 150 pesos for the orange patient sheet.
  5. Proceed to ARG and await for your name to be called.
  6. CD4 testing for positive patients are available after 1:00 P.M except during Wednesdays and weekends.
  7. Distribution of antiretroviral drugs are given monthly. Maximum supply for 1 month is dispensed at the given schedule.
  8. Drugs should be free and is subsidized by Philhealth.

The Thai Red Cross Research Center

If you have the budget of about 25,000 pesos every 6 months, a possible alternative is getting tested and treated in Thailand. The Thai Red Cross Aids Research Center is one of the most advanced HIV treatment facility in the world eclipsing RITM. They use the most advanced drugs that you need to only take once a day as compared to a cocktail of 2-3 medications that the Philippines still uses. Blood works such as CD4 testing is around 500 pesos and results are ready in 3 days. The medications used in TRCARC are the best medications available and is very cheap, it will only cost you about 10,000 pesos for your 6 months supply. This same medication costs about 80,000 per 1 month supply in the United States. It is cheaper in Thailand because it is subsidized by the Thai Government. You can only purchase 6 month supply of the drug as they will require you to come back every 6 months for monitoring your health status. This advanced medication is only taken once a day and has very little side effects. They look like Centrum tablets.

    Accurate naman cguro ung hiv test ko sa med ko for overseas.ang issue kc skin ay nong nag trangkaso after nong med for seabase then nag pacheck up ako sa labas unang check up ko uti savi sa lab.       pag mataas po ang wbc sa ihi it means meron kang UTI… na gamot mo ba yung tulo mo completely? bakit mataas parin sa ihi ang wbc mo, kindly have a rechecking sa gonorrhea.

      hindi totoo na hindi mawawala ang tulo pag nagkaroon ka na.. mawawala po ito. bacterial infection lang ito na kumakapit sa urogenital tract hindi pwepwedeng hindi yan mawala.

      single intramuscular dose of ceftriaxone 250 mg plus either a single dose of azithromycin 1 g orally or doxycycline 100 mg orally twice daily for 7 days ang drug of choice.. then mag papa check up ka after 2 weeks to check ulet kung meron.

      yes tama po iyon… na diagnose ako 2 years ago lang pero I was infected when I was 16-19 years old.

      nag base kasi sila sa cd4 ko and viral load… they said I am so very lucky dahil hindi pa naging AIDS yung HIV ko inspite na napakataas ng viral load ko and napaka baba ng cd4.

      they have ways para ma estimate when ka na infect base sa VL and CD4 as of the current so they estimated na 16-19 ako na infect since my infection is estimated to be 8 to 11 years ng active.

      HIV strain 1 pala ako.. hindi kasing agressive ng strain 2.

      nopes impossible po na 7 months hindi parin na detect, nangyayari lang yan sa mga taong may impaired immune response kung saan hindi nila inaacknowledge yung disease so hindi sila nag proproduce ng antibody. Im sure nung nag pa test ka sa hospital tinest pati yung virus mismo. Natural lang na pag gising mo sa gabi may manhid na part because yung posture mo kasi naiipit yung nerves, thats normal.

    Hi guys! I bought the kit from http://www.hivtestkit.ph and I tested positive… medyo hindi ako makapaniwala because I tested for a job abroad and required and hiv testing and I really think na wala ako because I cant remember any risk exposure.

    I bought another kit…. from the same site which is hivtestkit.ph kasi I heard accurate daw ito according to this website… I tested positive again.

      mag wawaste time and money ka lang kaka test, that kit from hivtestkit.ph is very accurate… same as ELISA yung methodology nya and I also tested positive jan sa kit na yan.. its made in Japan, very accurate talaga sha.

      I am afraid to say na pag nag pa test ka sa hospital, positive ka din. Its quite normal that you are saying wala kang risk exposure because the first usual response of a newly diagnosis client is denial. It would be best if may makakausap kang counselor. Update us in case mag papa test ka sa hospital

    Hello Ms Grace and everyone here. I’m 24, bottom gay guy, and I can say very promiscuous. I had so many oral sex encounters since 2010 until Feb 21, 2014. Ako most of the time ang sumusubo. Most of my encounters pinakamatindi na yung oral sex ang nangyari. Three times na din ako nagpa test within this period and negative naman lagi result.

    All my life, I tried to keep my back virginity intact until Feb 13 (or 14), 2014, for the first time in forever, may isang guy na nakapenetrate sa kin pero it was protected naman. I had my test last March 25, 2014 which turned out negative.

    Nov 27, 2014, I had protected anal with another guy. Until 2014 ended and never pa ako nagkaroon ng unprotected experience. Puro oral sex and two protected anal sex. Although may mga oral encounters ako na I let the guy na labasan sa loob ng bibig ko and I swallow the sperm, wherein karamihan ay noong 2014 nangyari.

    Jan 3, 2015. I was having anal with another guy. Eto lang talaga yung very brief encounter ko na I deviated from the rule and somewhat had it unprotected; it was “dipping” o pinasok lang nya ng ilang seconds yung kanya sa akin then pinahugot ko agad kasi kinabahan talaga ako. As in nagpumiglas talaga ako. Wala pang 10 seconds na nakapasok yun.

    After that night, I had hyperacidity and constipation. Siguro mga 1 week din ako hindi makadumi ng maayos. I have my regular daily bowel movement, pero I noticed that time, something changed. I took meds and drank pineapple juice and nakarecover naman ako from that.

    I also remember having phelgmy cough without colds for two weeks, which started around Feb 6 or 7, more than a month after that first, single risky encounter. Nung almost one week na hindi pa rin gumagaling yung cough ko, I self medicated na with Tuseran Forte, 3x a day for 7 days. I think last kong inom ng Tuseran was Feb 16 or 17. I recovered din naman, Feb 20 wala na akong cough.

    At this point, I still continue having oral sex with guys I meet/date. Until last Feb 21, 2015, after swimming in a jacuzzi sa isang spa, I noticed there were red spots on my hairline which really terrified me. I quickly searched online and I found it was more like seborrheic dermatitis. Di siya itchy, basta red spots lang. I also remember I started taking 50 mg zinc tablets on that same day, actually a few hours only, before I noticed the red spots on my hairline.

    Sa sobrang kaba ko and just to clear doubts, last Feb 24, I went to a Social Hygiene Clinic to get tested. I read kasi online na isa sa mga symptoms ng HIV ang seborrheic dermatitis. And to my surprise, REACTIVE ang result ko sa screening test which looks very similar sa nabibiling kit.

    Now, my questions are:
    1. With just that single, unprotected, anal dipping encounter, posible ba talaga na mahawa na agad ako? Napakasaklap naman at natiyempuhan talaga ako nung small percentage ng risks na yun given that less than 10 seconds na nakapasok yung junior nya sa kin. Possible din kaya na effect ito sa mga man juices na nilunok ko nitong 2014?
    2. Is there a big chance of a false positive in my case? Medyo nagdududa din kasi ako. Una dun sa pagkuha ng blood sample. The staff/nurse just pricked my finger tapos dinutdot yung daliri ko dun sa kit. Pangalawa, di kaya may effect yung pagkakaroon ko ng cough and pag-inom ko ng Tuseran Forte a few days before the test? Isa pa, di kaya naka-affect din yung parang seborrheic dermatitis sa ulo ko? Kasi parang fungal/yeast infection siya. Hindi kaya yung antibodies na nakita nung test method na ginamit nila eh dahil dun sa mga recent ailments ko na yun. Also, factor din kaya yung zinc supplements na sinimulan kong itake. Just saying the factors that I can think of.

    Haaayy. Sobrang nakakagulat, nakakalungkot, sobrang depressing. It will take 30 working days pa daw for the confirmatory testing to come out. Ifo-forward daw sa SACCL sa San Lazaro yung another blood sample na kinuha sa kin. Isang buwan ng agam-agam, isang buwan ng pag-iisip kung “Paano kung maconfirm ngang positive ako?”

    I need emotional support. Parang di ko kaya sabihin sa family ko itong naging findings sa kin. Natigil na lahat ng casual sex encounters ko, kahit oral sex, as in wala ngayon.

    Hindi ko na alam gagawin ko. Gulung-gulo na isip ko. Ang hirap magpanggap na masaya kahit sobrang bigat ng dinadala ko ngayon. Ayoko ipahalata sa kanila na may pinagdadaanan ko.

      1. hiv is not transferred sa oral sex unless puro sugat ang bunganga mo and nilabasan ka ng saksakan ng dami ng ejaculate sa bibig… wala pang documented case na na transfer ang hiv sa BJ.

      2. ang pagkakamali mo nakapasok miski 10 second, or 1 second, the mere fact na may precum at may maliit na tear sa rectal/anal lining mo, its enough to cause an infection.

      3. the symptom that night is not related, hindi ganong kabilis ang HIV, it will take YEARS for the symptoms to develop.

      4. yung symptom 1 month after that incident is a possible sign of acute infection, hence… possible na its brought about by HIV.

      5. I suggest retesting… get the kit from hivtestkit.ph or mag pa test ka either st lukes, ritm, san lazaro, medical city or makati med… wala ng iba pa.

      now to answer your questions:

      1. YES. lalo na bihira ka nakikipag anal meaning, masikip and mataas ang chance ng pagkapunit… so YES. Ignore yung mga oral sex mo, hindi ka ma iinfect ng HIV sa oral sex… walang documented case.

      2. HIV tests antibodies, hindi mag fafalse positive unless nag tatake ka ng hiv meds, mataas HCG mo or may certain viral disease ka.

      3. if ever infected ka nga, i would assume hindi lang once ang anal sex mo na unprotected… that is so very unlikely kasi but as i said, possible. Kindly try to recall other incidents like…. those you think na protected, condom could break and some idiot could intentionally remove it to infect you at hindi mo alam.

    hi tanonh ko lang po kung saan pwede mag pa hiv pcr dna test dito po sa philippines? kasi i had my exposure last aug 15,2014 then last december i had colds,coughs,headache and vomited one time plus a rash on my stomach. i tested last jan20,2015 non reactive bia sd bioline 3.0 kit sa hi precision diagnostics, then jan 28-feb9 se thing colds,cough,headache tested again last feb10 non reactive. ngayon takot na takot na ako. my test was taken 5 months an 5 days after exposure yung first non reactive. then was 180days after exposure nom reactive. nattakot lang po ako kasi i might be experiencing delayed seroconversion. by the wayy exposure was protected with csw botj oral and vaginal, kaso natatakot ako kasi may open wound ako sa groin area na baka napasukan ng vaginal fluids nung girl……. kaya i want to make sure po sana kasi ang merun po sa ibang hospitals is yung hiv pcr rna test yung viral load count, ang hanap ko po sama is the pcr dna test which looks for the virus itself. baka kasi hindi pa nag rerelease ng anitbody katawan ko kaya lagi non reactive sa test. i wanna make sure po sana na wala po ako talaga hiv maraming salamat po

        you need not abstain, you only need to have protected sexual intercourse para walang risk to make sure na hindi ka mahahawa during the testing period.

        example, 3 months na wala kang sex.. nag test ka nag negative, it means negative ka.

        but if nakipag sex ka 3 days ago, nag test ka tapos negative ka, hindi tayo sure kasi what if nahawa ka 3 days ago? gets? yun yung reason.. para lang alam natin

          Grace last sex ko feb 2014 tapos nag pa hiv test ako noong dec 2015 at negative naman accurate na kaya yon sa palagay mo ? Worried kasi ako ???

      Chemluminescence assay is the same as ELISA meaning, its 100% similar dun sa kit na nabibili from http://www.hivtestkit.ph chemluminescence assay din yun its a test that tests for the antibodies present sa dugo.

      yes nag pa second opinion ako sa medical city i think i paid 1200 they used Elisa, positive nga talaga pero i regret na nag pa medical city ako kasi na record na ata ako sa DOH ever since kasi nag pa check ako med city eh may mga nag tetext na sakin about seminars eh.
